GDPR was introduced on 25 th May 2018. Hence, in accordance with ICO (Information Commissioner’s Office) regulations, I need your agreement in order hold your personal information i.e. Name; email; telephone number; address, in case of emergency numbers (ICE).

The rationale for keeping your details is in order for me to contact you and for you to contact me. Also for the following reasons:

• In case of emergency, yours or mine and this particularly relates to the ICE details.
• The event of my sudden illness or death, when a trusted partner will contact you.

Your information is held securely and confidentially. Unless legally required to do so, or by your request, I will not pass your information onto a third party.
